Annual Physical Medical Examination
Dear All,
We'd like to announce that we will be conducting our Annual Physical Medical Examination that will run from November 18, 2024, to February 28, 2025, with our partner HMO provider Intellicare.
This is required for employees with active account number in Intellicare. Please see the attached file to check if you are included among the required employees.
Employees within Metro Manila must conduct their APE at the nearest Aventus Medical Care Clinics. To book your appointment, visit their website: Aventus Medical Appointment
For those residing outside Metro Manila, kindly fill up this Google Form: APE - For Non Aventus Clinic and we'll be coordinating your schedule and location, with a partner clinic/hospital near you. You will be receiving an email from the Human Resources Department once we got your schedule and assigned clinic.
For employees who will get their appointment on a weekday, you are entitled to a half-day off. (Kindly file under Official Bussiness (OB) in Sprout). Employees who will choose a weekend appointment, are entitled to an additional half-day pay. Kindly file using manual OT form (Additional pay will be based on your hourly rate). Also, kindly inform your immediate superior of your scheduled appointment especially if it is on a weekday.
