POST AND CORE IN ADDITION TO CROWN, INDIRECTLY FABRICATED


Post and core are custom fabricated as a single unit.



WARNING

It is inappropriate to report the prefabricated post and core (D2954) as an indirectly fabricated or cast post and core (D2952).


WATCH

1. The same code reports a post and core, indirectly fabricated (D2952) under either a single crown or the retainer crown of a bridge.

2. The fee for a post and core (D2952) is often higher than a prefabricated post and core (D2954).

Enter the tooth number being treated in Box 27 of the 2024 ADA Dental Claim Form.

LIMITATIONS

1. Some payors may reimburse the alternate benefit of a prefabricated post and core (D2954) for the indirectly fabricated post and core (D2952).

2. For anterior teeth with root canals, documentation of insufficient tooth structure to support a crown may be required to justify reimbursement for both D2952 and D2954.


NARRATIVES

1. The indirectly fabricated post and core (D2952) is generally reimbursed for a posterior tooth. The reimbursement for an indirectly fabricated post and core (D2952) is typically higher than for a core buildup (D2950) or the prefabricated post and core (D2954). A narrative may not be required when performed on a posterior tooth as it is assumed that the indirectly fabricated post and core is performed on an endodontically treated tooth. The indirect cast post procedure is used less frequently than the prefabricated post and core (D2954). A diagnostic image of the endodontically treated tooth should be submitted. Photographs may be helpful to document need. Payors generally require the endodontic treatment be clinically acceptable and the tooth free of pathology (endodontic and periodontic).

2. If tooth does not have existing RCT (root canal therapy), at the time of the predetermination, state when RCT will be done.

3. Estimate and state percentage of healthy tooth structure left after fracture, caries and/or any previous restoration is removed. "See attached radiographic image." (Send PA radiographic image).
